IF YOU DONATE ONLINE:
* Make either a one-time or a Monthly Giving gift of $50 or more
* After making your gift, return to this page and, using the form
below, fill out your first and last name (as it is on your credit card and
your e-mail address.
* Then, in the "message"part of the form, give us this information:
1) The amount you donated
2) The Confirmation Number from your transaction e-receipt
3) Full shipping address of where you want your gift sent
IF YOU DONATE BY CHECK:
* Your check of $50 or more that is post-marked by February 29, 2012
* Include a note with your check with your e-mail address or
phone number and with the address you want your gift shipped to
INTERNATIONAL DONORS WHO WANT YOUR GIFT SENT TO AN ADDRESS IN THE USA:
* Follow the steps for online donations above but, instead of your
mailing address, give us the name of who you want your gift shipped to
and their mailing address
